This form must be completed thoroughly and honestly, and signed by the athlete’s parent or legal guardian. This form applies to the 2026-2027 season and needs to be submitted no later than August 31, 2026. This form and its contents will be available to authorized STARZ Cheer NJ personnel and kept confidential.
